two days to tues day

04.12.09

do you believe it?
do... you... fucking believe it!?!
we are here.

In two days... we'll still be here.
But there
in the crowds, just beyond those clouds
blowing funny shapes
for the silly prime mates.

To gaze or stare
or let loose that burden to bare.
To learn and teach
To teach and learn.
To love and touch
it's never too much.

Abruzzo's uprising,
yet we are still
up and rising.
hour to hour. second to fucking millisecond.
Let ours create
days that sing, and nights that moan.
Baby, I'm ready for you to come home.
I’ll see your holstered silence
and raise you one.
Seven hundred and thirty days
when my soul was shunned.
You still don’t know?
It’s not supposed to be all hustle and flow.

Broadsided, not by my fist
nor a bus
nor an illusion of us.

Invincible illustration
lacking proper dictation
dedication.

We drift on in calmer tides.
Wet emerald moss on soggy
tattered
rafty
remains.
Drunken from a year at sea.
Is that an elephant in the billowy white looking down at me?

Illusion or confusion.
Gots to keep it movin’.
I heard about a place in Berlin, where it’s ok to raise children.

You haven't heard?
mums the word.

Save your sympathy.
Humble yourself instead.

1900-0730: if all goes well

1900-1915
get assignment
1915.01-1915.99
pray that everyone is stable
1916-2000
make sure everyone is alive and relatively happy
get report from out-going nurse
follow-up on immediate patient needs
2001-2045
review patient history, medications, labs, tests, x-rays, new orders, patient care plan
2046-2100

2101-2245
full body assessments
1st round of med passes, stock rooms, change tubing as needed
2246-2250
make first pot of coffee
breathe
2255-0015
sit down at computer with a hot cup of coffee
chart Acuity (software they use that tells the bosses how much time each patient requires) (mandatory)
chart everything I did in the first four hours (also madatory)
0016-0100
check blood sugars and give insulin as needed
2nd round of med passes
0100-0115
make second pot of coffee
breathe
0115-0400
baths and wound care
0401-0500
replace tube feedings
replace IV bags
make sure everything is ready for the next shift
0501-0615
3rd round of med passes
2nd round of blood sugar tests
0616-0715
chart what I've done all night
prepare report for on-coming shift
0715-0730
give report
0731
breathe

It's just a matter of basic math

12 hour shift-4 hours of menial work that has no relevance to patient care =8 hours for patient care

8 hours/8 patients=1 hour per patient
